Patient Report Form -
a strenuous, carbon-copy three-paged form, which is filled out when we have a casualty in our midst. Although it is relatively straightforward, the thing I hate most about doing one is actually spelling correctly. When you have a casualty who is unable to write and you have to fill in their personal details for them - and they mumble a lot - then it can be pretty difficult to catch the right address or even name.
And then you have to keep asking them.
And then you just feel stupid.
